CHRONIC MEDICAL CONDITIONS

If your child has a chronic health condition (asthma, seizures, diabetes, food allergy, etc.), please fill out the appropriate Health Plan form/s listed below. 

Per CHCCS policy, all Health and Emergency Actions Plans need to be updated each school year.  If you have completed a form this school year, you do not need to take any further action. If you have not completed a form for this school year, please complete the appropriate form. Make sure the forms are signed by your medical provider and then send the form/s to Nurse Quinn at qcampbell@chccs.k12.nc.us

Prior to returning to the school building, after-school activities, and athletic events:

• you must have no fever for the previous 24 hours (without fever-reducing medication/s); 

• your symptoms must be improving and you're feeling well enough to attend school all day; and 

• no vomiting or diarrhea for the previous 24 hours (without medication). 

Upon your return to school, please remember to wear a mask for the following five (5) days.
